Disability Exams

Valor 4 Vet offers in person disability examinations, or Face to Face examinations, for completion of Disability Benefits Questionnaires. If an examination is required, an in person examination is the only way to provide an appropriate assessment of the disability for several reasons.

A Mental Health Expert will review the medical records to assure the necessary information is available to complete the federal forms.

4. Schedule

Once approved, the office will  get the veteran scheduled with the Mental Health Expert.

Fill out a Face to Face Exam form in the Portal. A Medical Examiner will review the request and approve the request.

Our Mental Health Expert will examine your records to confirm they contain the details needed for federal forms.

After the review has been approved, our office will coordinate to schedule your video appointment with the Mental Health Expert.

For access to our online exams, please sign into the portal.

A fee of $150.00 is required prior to starting the exam.

Our Medical Expert will review your provided evidence and may contact you for further questions. Expect to receive your completed DBQ within 5-7 business days.

*The Veterans Administration Regional Office, or VA Regional Office, has placed restrictions on disability examinations conducted by Non-VA Medical Providers. Read more about those restrictions in the M21-1 here. The Board of Veterans Appeals is not bound by the M21-1.
