Disability Benefits Questionnaires (DBQs) are used to assess the current state of a veteran’s disability. The DBQs are designed to provide a disability percentage rating associated with the disability. The Acceptable Clinical Evidence process, or ACE process, developed by the Office of Disability and Medical Assessment, allows medical examiners to complete the DBQs based on the existing medical evidence in the veteran’s medical record, along with an interview.

Valor 4 Vet has a limited number of online DBQs available using the ACE process.

It’s important to note that the Veterans Administration Regional Office (VA Regional Office) has imposed restrictions on disability examinations conducted by Non-VA Medical Providers.

For more information on these restrictions, refer to the M21-1.

The cost of each examination by Valor 4 Vet ranges from $150.00 to $200.00, payable prior to accessing the questionnaire. A Valor 4 Vet Medical Expert will review your medical record, your interview questions, and may reach out with additional questions.

A completed DBQ will be delivered within 5-7 business days.

We currently offer a limited selection of disability exams online, including Chronic Fatigue Syndrome, Erectile Dysfunction, Esophageal Conditions, Headaches, Irritable Bowel Syndrome, and Sleep Apnea.
